The patient must be homebound
The patient may be able to leave home only with great difficulty, infrequently, and for a very short duration.
Candidates for Home Health Care
If homebound,
Having one or more of the following conditions may potentially make the patient a candidate for home health services:

OUR HOME HEALTH SERVICES
SKILLED NURSING
Our nurses work with your doctor to provide home care services. Their assessment, skilled treatment, & support provide the high quality care you need to recover at home.
HOME HEALTH AIDES
Assist with activities such as grooming, general hygiene, light meal preparation, eating assistance, and also assistance with daily living activities, light housekeeping, and ambulation assistance. Attention to personal needs is vital component of our services.
HOME THERAPISTS
Our physical therapists, occupational therapists, and speech language therapy professionals provide complete rehabilitation needs assessment, home safety evaluation and training, training for strength and mobility, activities of Daily Living training, retraining for language and communication skills, as well as home medical equipment referrals.
MEDICAL SOCIAL WORKER
Help patient/family members understand/cope with problems caused by illness, personal/family issues, alternative care planning, community resource referral, counseling for long-range planning and decision making.
